Before an adoption is completed we will ask you to fill out the following contract. We encourage you to review all the terms if you are seriously considering adoption. However, please don’t complete the form unless requested by a Mile High Dachshunds Rescue Representative. Thank you!

The primary adopter agrees to be bound by the following conditions and further agrees to assure all members of his/her household agree to the same. The adoption fee will be used by Mile High Dachshunds Rescue (MHDR) to offset costs incurred in rescuing this dog and in caring for other animals in the future. This adoption will not be deemed completed until the total amount of the adoption fee is paid. The adoption fee is not returnable after a seven-day trial period. Adoption fee for adult dogs is $350.00. For puppies through 12 months of age, the adoption fee is $400.00.(Required)

1. To have a good quality of life, the adopted dog must have all Five Freedoms: Freedom from Hunger and Thirst; Freedom from Discomfort; Freedom from Pain, Injury, and Disease; Freedom to Express Natural Behaviors; and Freedom from Fear and Distress.(Required)

2. If I can no longer care for the adopted dog, I will notify MHDR and will relinquish the dog to an MHDR representative immediately. The adopted dog cannot be transferred to any person or entity other than MHDR.(Required)

3. I understand that future veterinary medical problems may arise with this dog including serious illness requiring emergency care or surgery. Those veterinary medical problems are my responsibility and I will bear all related costs and will provide the proper veterinary medical care to the dog in a timely manner.(Required)

8. Ownership of a dog poses dangers to people and animals. Such dangers include, by way of example and not limitation, the risk that the dog can bite, trip, and knock down people, especially children and the elderly or infirm, and can fight other animals.(Required)

9. We MHDR has limited information about the dog, and therefore cannot predict whether the dog will be suitable for my lifestyle and household. Accordingly, as the Primary Adopter, and on behalf of my spouse/partner and all minor children assume the risk of injuries, losses, damages, costs, and expenses incurred by any means above described, and other injuries, losses, damages, costs and expenses of every possible cause and description caused by the dog, and agrees that MHDR shall not be held responsible for the same.(Required)

This agreement supersedes all prior discussions, representations, warranties, and agreements of the parties, and expresses the entire agreement between the Primary Adopter and MHDR regarding the matters described above. The parties confirm that, except for that which is specifically written in this agreement, no promises, representations, or oral understandings have been made with regard to the dog or anything else. Without limiting the generality of the foregoing, the Primary Adopter acknowledges that MHDR has not represented, promised, guaranteed, or warranted that the dog can be trained; that the dog will never bite; that the dog will not be dangerous or vicious in the future; or that the dog will not exhibit other behavioral problems. This agreement may be amended only by a written instrument signed by both the Primary Adopter and MHDR. Any remedy provided in this agreement is in addition to any and all other remedies provided by law or equity. If any provisions of this agreement are invalid, void, or unenforceable, they will be severed and all remaining provisions shall be given full force and effect.(Required)

Written disclosure of all treatments received by the dog while in the care of MHDR involving immunizations, medications, and any other veterinary treatments administered prior to the date of the dog's transfer to the Primary Adopter.(Required)
